Kevin Polky
CADC, LCSW | FOUNDER & PRESIDENT
With more than 30 years of experience under his belt, Kevin Polky is amply qualified to oversee Four Corners Wellness Center, ltd. as both founder and executive director. He is a licensed clinical social worker, certified alcohol and drug counselor, certified school social worker, and executive coach.
Sonya Mcdevitt
CADC, NASM-CNC, LCPC | NUTRITIONAL COACH & EMDR TRAINED
Sonya is a Licensed Clinical Professional Counselor (LCPC) in both Illinois and Wisconsin, a National Certified Counselor (NCC), a Certified Nutrition Coach (NASM-CNC), and is trained in EMDR. She completed her Master’s degree in Mental Health Counseling from Walden University.
Jim Johnson
CADC, LCPC | Executive Coach
Jim graduated with his Bachelor of Science degree from the University of Illinois and completed his Master’s Degree in Counseling at Northern Illinois University.
Cody Berillo
LCSW | GRIEF COACH
Cody Berillo earned her BS in Family and Child Studies from Northern Illinois University and has her MSW from Aurora University.
Pat Spangler
LPCP, CADC, MAATP
Pat received his M.S. Ed at Northern Illinois University and is a Licensed Clinical Professional Counselor in Illinois. He also is a Certified Alcohol and Other Drug Counselor (CADC), as well as a Medication, Assisted Addiction Treatment Professional (MAATP).
Arianna Calkins
LCSW | Equine Specialist
Arianna earned her Bachelor of Arts in Psychology from Northern Illinois University in 2017 and her Master’s in Social Work from George Williams College of Aurora University in 2020. She is a licensed clinical social worker with experience working in a clinical setting since 2020, focusing on clients aged 14 and up.
Kaari Hobson
LSW
Kaari Hobson is a clinical mental health therapist and Licensed Social Worker (LSW) who has worked in the behavioral health field since 2024. She earned her Masters of Social Work at Aurora University with a focus in Clinical Mental Health and military and veterans.
Nick Votava
LCSW
Nick is a Licensed Clinical Social Worker, holds a Professional Educators License, and has over 12 years of experience working with children and families in the Rockford region. Nick has worked within the Rockford Public Schools and Harlem Public Schools as a Truancy Interventionist, School Social Worker, Counselor/ Therapist.
